29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about August 25, 2020, in the workshop, employees engaged in pulling off steel coils from the slitter machine were exposed to crushing hazards.  The employer did not ensure a specific hazardous energy control procedure was developed, documented, and utilized.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  On or about August 25, 2020, the employer did not report an employee's work-related inpatient hospitalization to OSHA within twenty- four (24) hours.  The employee sustained multiple finger injuries from a crushing injury.  The employer reported the hospitalization on August 27, 2020.
