29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):    On or about September 10th, 2020, at The Shops at Richland Creek jobsite in O'Fallon, IL, employees working in an excavation at a depths of 5.5 to 6.5 feet were not protected from cave-ins through the use of sloping & benching systems, support shields, shield systems, or other such protective systems.
