29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   (a) 3105 W Liberty St.  Allentown, PA.: On or about September 9, 2020, an employee was not wearing head protection while working in a trench that had spoil piles of dirt and large rock within two feet of the trench's edge.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph, or video evidence of abatement or other written records.

Abatement certification and documentation required within 10 days after abatement date.
