29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about September 15th, 2020 an employee was working in an excavation in excess of five feet performing water line installation work and was exposed to cave-in hazards due inadequate protective system was not being used.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On September 15th, 2020, an employee was performing water line installation operations in a trench using a reciprocating saw without the use of eye protection.  The Guardian Utility Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 C.F.R. 1926.102(a)(1) which was contained in OSHA inspection number 1353790, citation number 1, item number 1 and was affirmed as a final order on 12/13/18, with respect to a workplace located at North Quaker Ave near Ursiline, Lubbock TX 79401
