29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a)  On or about September 15, 2020 employees at the site were exposed to amputation hazards when the saw blade guard on the Makita circular saw was wedged back with a piece of wood preventing the guard from functioning.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about September 15, 2020 at the site employees were exposed to fall hazards greater than 6 ft. while framing up the rafters on the single story residence without fall protection.
