An inspection shall be conducted by the competent person prior to the start of work, as needed throughout the shift, and after every rainstorm or other hazard increasing occurrence: (a) On or about September 15, 2020 - 1322 Pratt Avenue NE, Huntsville, AL, employees working in an excavation approximately 5 or more feet deep with additional depth of approximately 2 - 2 1/2 feet deep footings were exposed to struck-by hazards when inspections by a competent person were not performed.
