29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or about September 17, 2020, five employees were working at replacing an asphalt shingle roof and none of the employees were protected from falling to the ground.  The fall hazard ranged from about 9 feet to 18 feet.  The pitch of the roof was 5 in 12.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:  (a)  Jobsite:  On or about September 17, 2020, the 5 employees that were working on a roof and were not using fall protection need to be retrained on when and how to properly use fall protection when working on a roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  Jobsite:  On or about September 17, 2020, employees were using an aluminum extension ladder that only extended about 1 foot above the edge of the roof.   The fall hazard to the employees was about 9 feet.
