29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this standard:     a) On or about September 16, 2020 two employees working in an excavation up to 11 feet deep were not protected by cave-in hazards from the south wall while working on the installation of the line-stop for the 16 inch water main.     b) On or about September 16, 2020 two employees were not protected from cave-in hazards up to nine feet 4 inches when they exited the trench box to access the egress ladder on the east side of the excavation.
