29 CFR  1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees; without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.   On or about September 17, 2020 and at times prior thereto, the employer allowed employees to use A-frame ladders on the platform of a scaffold.  The ladder was not secured and exposed employees to a fall hazard.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation.  On or about September 17, 2020 and at times prior thereto, the employer did not ensure that the base plates were installed on the scaffold end frames.  Employees were working from the scaffold and were exposed to a fall hazard.  a.  The scaffold located on the east side of the front entrance overhang. b.  The scaffold located on the west side of the front entrance overhang.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about September 17, 2020 and at times prior thereto, the employer did not ensure that employees, performing roofing activities, were in use of fall protection.  This exposed employees to a fall greater than six (6) feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed and the ladder was not secured.   On or about September 17, 2020 and at times prior thereto, at the southwest side of the building near the front entrance, the employer did not ensure that the side rails of a ladder extended at least 3 feet above the landing.  The ladder was not secured and exposed employees to a fall hazard.

On or about September 17, 2020 and at times prior thereto, the employer did not ensure that A-frame ladders were used in a closed position and leaned against the building.  This exposed employees to a fall hazard.  a.  On the southwest scaffold platform, an A-frame ladder was being used in the closed position and leaned against the wall. b.  On the southwest scaffold platform, an A-frame ladder, that was in the closed position and leaned against a wall, was being used to access the roof.
