29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) On or about 9/16/2020 at a project located at 374 Fluvanna Avenue, Jamestown, New York; employees were using a boom lift to access work areas while performing roofing work on the rear of the building. The employees in the lift did not have harnesses with lanyards attached to the boom or basket. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(10): Employee(s) engaged in roofing activities on low-sope roofs, with unprotected sides and edges 6 feet (1.8m) or more above lower levels were not protected from falling by a guardrail system, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system(PFAS), or warning line system and safety monitoring system: a) On or about 9/16/2020 at a project located at 374 Fluvanna Avenue, Jamestown, New York; employees were installing a metal roof and accessories (cupola) on a new storage building. The roof had a 4 in 12 pitch and employees were not using PFAS or any other method of fall protection. Employees were exposed to approximately 24 feet fall to the ground. NO ABATEMENT CERTIFICATION REQUIRED
