29 CFR  1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;    On or about September 18, 2020 and times prior thereto, employees perform maintenance and servicing on the press machine and the lockout/tagout procedures to de-energize moving equipment did not provide location of the disconnect while employees perform maintenance and servicing on the press machine.  Employees were exposed to caught-in and struck-by hazards.

29 CFR 1910.217(h)(10)(ii)(D): A check shall be made to assure that the barriers and/or supplemental presence sensing devices required by paragraph (h)(9)(ix) of this section are operating properly.   On or about September 18, 2020 and times prior thereto, the employer did not perform inspection on the barrier located on the two hand press machine.  The curtain is in need of repair or replace to prevent exposing employees to caught-in, struck-by, and/or crushed-by hazards.
