29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) SU Dome roof, north side, west end, on or about 9-15-20 and 9-16-20:  Employees were working on a sloped membrane roof, without the specific training necessary for the employees to recognize the hazards associated with falls and their work, exposing the employees to a sliding hazard of upwards of 100 feet and a fall hazard at high speeds of 4 feet 4 inches.  ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM

29 CFR  1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) SU Dome, north side, west end, Buck Hoist area, on or about 9-15-20 and 9-16-20:  Employees were accessing a Buck Hoist using a stairway without a stair rail on the open side of the stairs, exposing the employees to a fall hazard of up to 26-1/2 inches.
