29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:  (a) On or about October 2, 2020, an employee working off extensions on the second tier of self-supported tubular welded end frame scaffolding was not protected from a fall hazard of approximately fourteen (14) feet in that the scaffolding was not fully planked.  (b) On or about October 2, 2020, an employee working while standing on the outer edge of offset planks on the second tier of self-supported tubular welded end frame scaffolding was not protected from a fall hazard of approximately twelve (12) feet in that the scaffolding was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access:  (a) On or about October 2, 2020, at least three employees were accessing self-supported welded tubular end frame scaffolding by using the end frames and cross braces exposing them to a fall hazard of approximately ten feet or more to the ground below.

(b) On or about October 2, 2020, an employee was doing masonry work using the lower roof as a working platform with no fall protection and was exposed to a fall hazard of approximately ten feet to the ground below.
