29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval: Note:  If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly.  Employees were using the All Terrain Forklift, Skytrack, Model 10054 and a synthetic web sling placed directly onto the tines to make a below-the-tines lift of a 1,500 pound, 25 foot long steel column.  Employer did not ensure the forklift had an approved lifting point, thereby exposing workers to struck-by and crushing hazards.
