On or about October 1, 2020, at a work-site 1142 Lakeview Ave NW Canton, OH 44708; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 12 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 1, 2020, during the installation of roofing material at a job site located at 1142 Lakeview Ave NW, Canton OH 44708; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 12 feet by utilizing a fall protection system.
