29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  a) Jobsite, 420 Mableton Avenue, Mount Lebanon, PA 15228: On or about October 07, 2020, during excavation activities, the employer did not ensure that an adequate cave in protective system was in place, in there was no cave in protection inside the trench that was measured to be 5 feet 4 inches in depth.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.  Job site, 420 Mapleton Avenue, Pittsburgh, PA 15228: On or about October 7th 2020, employees working in the 5' 4" trench were using a A-Frame ladder in a manner not approved by the manufacturer, in order to get in and out of the trench.
