29 CFR 1910.30(a)(1): The employer did not provide training for each employee who used personal fall protection systems or who was required to be trained as specified elsewhere in this subpart before the employees were exposed to a fall hazard. (a) On or about October 7, 2020, the employer did not ensure employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work. This exposed employees to a fall/thrown hazard of approximately 18 feet.

29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift. (a) On or about October 7, 2020, an employee was working from a Genie Z-30/20N aerial lift replacing lights without the use of a safety harness and lanyard attached to the lift. This exposed employees to a fall/thrown hazard of approximately 18 feet.
