29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns were not protected by protective helmets:  On or about October 07, 2020, and at times prior thereto, at a workplace located at 2661 Stadium View, Fort Worth, Texas; at the new residential construction site, employees walking and cutting framing material below workers was exposed to the hazard of falling tools and materials and was not wearing a hard hat.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.    On or about October 07, 2020, and at times prior thereto, at a workplace located at 2661 Stadium View, Fort Worth, Texas; at the new residential construction site, employees were walking and working from the roof and not protected from falling more than 14 feet to the lower level by use of guardrail or personal fall arrest system.
