29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  North side of building.  On or about October 7, 2020, employees were exposed to a fall hazard of greater than 6 feet from the ground level below, while attaching metal roofing strips, without any form of fall protection.

29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder while employees accessed the roof.  North side of building.  On or about October 7, 2020, employees were exposed to a fall hazard while accessing the roof from a ladder whose foot was not one-quarter the working length horizontally away from the roof edge.
