29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 10/8/2020, at the front and back of the house located at 1235 Mayfair Drive, Brookhaven, GA 30329:   a)Employees performing framing work while using a 36 ft extension ladder extended about one (1) foot above the upper landing were exposed to a fall hazard at about 15 feet from the ground level.  b)Employees performing framing work while using a 18 ft extension ladder extended about two (2) feet below the upper landing were exposed to a fall hazard at about 15 feet from the ground level.

29 CFR  1926.1053(b)(6):Unsecured ladder(s) were used on surfaces that were not stable and level:  On or about 10/8/2020, in the front, back, and side of the house located at 1235 Mayfair Drive, Brookhaven, GA 30329:  a) employees using 18 feet extension ladders placed on muddy, unstable, and unlevel surfaces while engaged in wood framing were exposed to fall hazards at heights about 15 ft from the ground.  b) employees using 28 feet extension ladders placed on muddy, unstable, and unlevel surfaces while engaged in wood framing were exposed to fall hazards at heights about 15 ft from the ground.  c) employees using 36 feet extension ladders placed on muddy, unstable, and unlevel surfaces while engaged in wood framing were exposed to fall hazards at heights about 15 ft from the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 10/8/2020, at the jobsite located at 1235 Mayfair Drive, Brookhaven, GA 30329, Employees engaged in wood framing work on residential homes without fall protection were exposed to fall hazards greater than 13 feet from ground level.   The employer failed to ensure all employees engaged in roofing activities were completely protected from any fall hazards.
