29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns were not protected by protective helmets:   On or about October 09, 2020, and at times prior thereto, at a workplace located at 360 Kirkpatrick Drive, Weatherford, Texas; employees walking and working below workers above them were exposed to the hazard of falling tools and/or materials and were not protected by the use of hard hats.

On or about October 09, 2020, and at times prior thereto, at a workplace located at 360 Kirkpatrick Drive, Weatherford, Texas; employees installing decking materials without fall protection were exposed to falling more than 24-feet to the lower level.
