29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.:  JOB SITE:  635 S. 6th Street, Poplar Bluff, MO 63901, on or about  10/15/2020, Inside of the Plasma Cutter Work Station, a Hypertherm Powermax 65, 220v Plasma Cutter was plugged into an extension cord. The flexible (extension) was extended about thirty (30) feet across the building and plugged into a single outlet near the Maintenance bay exit door.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap:  JOB SITE: 635 S. 6th Street, Poplar Bluff, MO 63901, on or about 10/15/2020, workers were permitted to operate a  Hypertherm Powermax 65 Plasma Cutter,  S/N: 65-017906 with a damaged power cord.  The power cord insulation had been repaired with black electrical tape to cover exposed wiring.  Workers were exposed to an electrical hazard.

JOB SITE: 635 S. 6th Street, Poplar Bluff, MO 63901, on or about 10/15/2020, the employer did not ensure connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  The following flexible cord was damaged.  a)	Inside of the Plasma Cutter Station, the power cord was used to power a 220v plasma cutter.  The outer jacket was damaged and provided no strain relief.
