29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) On or about October 8, 2020, and at times prior, the employer did not ensure that work rests on grinding machinery were adjusted closely to the wheel with a maximum opening of one eighth inch. Employees were exposed to struck-by hazards as follows:     1- In the front shop a Dewalt DW 756 8" bench grinder, S/N 9831TYL0166, was used while the work rest was not adjusted properly. The distance between the abrasive wheel and the work rest was three quarters of an inch.      2- In the SW area of the welding shop a Dayton 8" bench grinder, S/N 12707, was used while the work rest was not adjusted properly. The distance between the abrasive wheel and the work rest was one half of an inch.      3- In the SE area of the welding shop a Dayton 8" bench grinder, S/N 12708, was used while the work rest was not adjusted properly. The distance between the abrasive wheel and the work rest was three eighths of an inch.   Abatement Note: Abatement certification is required for this item. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about October 8, 2020, and at times prior, the employer did not ensure that the distance between the abrasive wheel periphery and the adjustable tongue guard did not exceed one fourth inch. Employees were exposed to struck-by hazards as follows:      1- In the front shop a Dewalt DW 756 8" bench grinder, S/N 9831TYL0166, was used while the distance between the abrasive wheel and the tongue guard was three quarters of an inch.      2- In the SW area of the welding shop a Dayton 8" bench grinder, S/N 12707, was used while the distance between the abrasive wheel and the tongue guard was one half of an inch.      3- In the SE area of the welding shop a Dayton 8" bench grinder, S/N 12708, was used while the tongue guard had been removed. 29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a) On or about October 8, 2020, and at times prior, the employer did not ensure that sufficient access and working space was provided and maintained about electric equipment operating at less than 600 volts. Employees were exposed to electrical hazards in that access to electrical panels was blocked by tools, equipment, and materials as follows:        1- In the South wall of the front shop access to an electrical panel was blocked by a hand notching machine.      2- In the SW area of the shop access to an electrical panel was blocked by a tool chest.
