29 CFR 1917.112(b)(1): Guardrails shall be provided at locations where employees are exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which present a hazard of falling more than 4 feet (1.22 m) or into the water, except as specified in paragraph (b)(2) of this section. a) Cement Terminal, on or about October 8, 2020, the guard rail was not constructed between the terminal and the gangway exposing employees to a fall hazard of 15 feet and the employer did not insure that guardrails were in place during inspections.

29 CFR 1917.154: Employees shall be protected by chip guarding and personal protective equipment complying with the provisions of Subpart E of this part during cleaning with compressed air. Compressed air used for cleaning shall not exceed a pressure of 30 psi. Compressed air shall not be used to clean employees. a) Cement Terminal Check In Station, on or about October 8, 2020, the air compressor produced at the nozzle 100.3 PSI exposing employees to eye and skin injury and the employer did not insure that air pressure was reduced to 30 PSI.
