Jin Zheng worksite located at 7340 W Central Ave, Toledo, OH, 43617:  On or about October 14, 2020,  the employer did not ensure employees were protected from head injuries while setting  a restaurant sign, tools and associated materials on top of ornamental frames located 10 feet above ground level.  Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.
