29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  a) 31094 River Rd. Orange Beach, AL, 36561:  On October 6, 2020, the employer had an employee performing roofing activities fall 18 feet, the employee suffered multiple injuries requiring they be hospitalized from October 6, 2020 until October 21, 2020.  The employer failed to report the work-related hospitalization to OSHA within twenty-four (24) hours.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) 31094 River Rd. Orange Beach Alabama 36561: on or about October 6 2020, the employer failed to provide documentation certifying that employees performing roofing work had received training in fall protection and the use of fall protective equipment.
