On or about October 14, 2020 on the roof of a residential home being constructed, employees was exposed to struck by hazards while operating a hand held saw without eye protection.

On or about October 14, 2020 employees were exposed to a fall hazard greater than 6 feet while performing framing work without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  On or about October 14, 2020 at the residential construction site where employees were exposed to a fall hazard while using an extension ladder to access the upper level without the ladders side rails at least three feet above the upper landing.
