29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a)  On or about October 15, 2020, and at times prior, the employer did not provide employee(s) with Appendix D of the respirator standard, which contains information for voluntary use, where employees were voluntarily using an N95 particulate filtering facepiece.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
