29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) Shavings Line - On or about October 15, 2020, the employer did not ensure that employees were protected from falls when working near the area on the North/South Conveyor where there were approximately two foot gaps in the guardrail. Employees were exposed to a fall of approximately eight feet. b) Newman In-feed Log Deck Conveyor ("Old" Newman Inside Log Deck)(Furthest from Operator's Booth) - On or about October 15, 2020, the employer did not ensure that employees were protected from falls when working from the area on the log deck conveyor where there was approximately six feet of no guardrail and/or catwalk. Employees were exposed to a fall of approximately 14 feet. c) Inoteck In-feed Log Deck Conveyor ("New" Newman Inside Log Deck) (Closest to Operator's Booth) - On or about October 15, 2020, the employer did not ensure that employees were protected from falls when working from the area on the log deck conveyor where there was approximately two feet of no guardrail and/or catwalk. Employees were exposed to a fall of approximately 12 feet. d) Newman Outside Log Deck Conveyor ("Old" Newman Outside Log Deck) (Furthest from Operator's Booth) - On or about October 15, 2020, the employer did not ensure that employees were protected from falls when working from the area on the log deck conveyor where there was no catwalk for employees to access and no fall protection in that area. Employees were exposed to a fall of approximately eight feet. e) Outside Inoteck Log Deck Conveyor ("New" Newman Outside Log Deck) (Closest to Operator's Booth) - On or about October 15, 2020, the employer did not ensure that employees were protected from falls when working from the area on the log deck conveyor without fall protection. There was a catwalk on one side of the conveyor for employees to access, but the other side of the conveyor there was no catwalk and or/guardrail in place. Employees were exposed to a fall of approximately eight feet.
