Location - 75 Phoenix Avenue, Lowell, MA 01852.    a) On or about December 20, 2021, the employer did not provide abatement certification regarding citation 1 Item 1, for a site specific health and safety plan (HASP) to address health and safety hazards and levels of respiratory and personal protection for affected employees during hazardous waste operations and emergency response phases, within 10 calendar days after the abatement date.  b) On or about December 20, 2021, the employer did not provide abatement certification regarding citation 1 Item 2, for adequately performing a site characteristic evaluation to aid in selection of respiratory and personal protection level(s) for employees, within 10 calendar days after the abatement date.  c) On or about December 20, 2021, the employer did not provide abatement certification regarding citation 1 Item 3, for providing training to employees engaged in hazardous waste operations and emergency response (HAZWOPER), within 10 calendar days after the abatement date.  e) On or about December 20, 2021, the employer did not provide abatement certification regarding citation 1 Item 4a, for conducting initial air monitoring prior to letting employees start HAZWOPER site operation(s), within 10 calendar days after the abatement date.  f) On or about December 20, 2021, the employer did not provide abatement certification regarding citation 1 Item 4b, for conducting periodic air monitoring of the work areas where employees were cleaning up uncontrolled release of chemicals, within 10 calendar days after the abatement date.
