The ladder was leaned against the first story framing in the closed position while in use. As a result, employee was exposed to falls and resulting injuries from the ladder due to improper and unsafe use of the ladder.

Located at 6890 Lobelia Drive, New Albany, Ohio: On or about October 21, 2020, an employee was utilizing a portable Werner 6-foot A-frame stepladder to access the trusses of the second-story of the residential structure being built. The top of the stepladder as well as the top step of the stepladder were used to access the above trusses. As a result, the employee was exposed to falls due to utilizing the stepladder in a way in which it was not designed.

Located at 6890 Lobelia Drive, New Albany, Ohio: On or about October 21, 2020, an employee was performing framing work from the bottom web of the first story trusses without fall protection, thereby exposed to an approximately ten foot fall hazard.  Tito And Edward Construction was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1273419, citation number 1, item number 1 and was affirmed as a final order on November 29, 2017, with respect to a workplace located at Woodsbury Landing, Lot 50, Courtyards at Bradford Court, Powell, OH 43065.
