First aid supplies approved by a consulting physician were not readily available:  a) On or about October 23, 2020 in the Twin Knobs Work Center maintenance areas employees were exposed to injuries including, but not limited to, laceration, burn, strain, sprain, broken bones and the employer did not ensure a person or persons were adequately trained to render first aid and first aid supplies approved by a consulting physician were readily available.

29 CFR  1910.243(e)(1)(i):Power lawnmowers of the walk- behind, riding-rotary, and reel power lawnmowers were not guarded in accordance with the machine guarding requirements in 29 CFR 1910.212, General requirements for all machines.  a) On or about October 23, 2020 at the Staunton Work Center maintained areas the John Deere Z915B (1TC915BCCGT041489) riding mower exposed employees to struck-by hazards and the employer did not ensure the chute guard was maintained as an effective guard.  b) On or about September 23, 2020 at the Staunton Work Center maintained areas the Bush Hog (Model: SQ48R-3, Serial: 1101155) mower exposed employees to rotational hazards and the employer did not ensure the shaft guard was maintained to protect employees.  c) On or about August 23, 2020 on the Twin Knobs Work Center maintained areas the Massey-Ferguson 1210 equipped mower exposed employees to rotational hazards and the employer did not ensure the shaft guard was maintained to protect employees.

29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about August 23, 2020 at the Twin Knobs Work Center garage employees performing metal grinding with the Delta (Model: 23-980, Serial: P9452) were exposed to struck-by hazards and the employer did not ensure the tongue guard was installed and adjusted to within one fourth inch of the wheel periphery.  Notice 1 #, Item 2B #, 29 CFR 1910.215(b)(9) as required by 29 CFR 1960.8(b): The employer failed to ensure tongue guards are adjusted to within one quarter inch of the wheel periphery. The employer was cited for a violation of the same standard and hazard on April 24, 2020, Inspection 1470401 #, date May 22, 2020 of final order/settlement, and means of abatement.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) On or about October 23, 2020 in the Staunton work center garage employees were exposed to electrical hazards from a four outlet conduit box providing electricity to battery chargers and the employer did not ensure electrical equipment was listed and labeled for the use.  b) On or about October 20, 2020 in the Staunton work center garage employees were exposed to electrical hazards from a four outlet conduit box providing electricity to a Northern Industrial retractable cord reel and the employer did not ensure electrical equipment was listed and labeled for the use.  Notice 2 #, Item 1 #, 29 CFR 1910.303(b)(2) as required by 29 CFR 1960.8(b): The employer failed to ensure electrical equipment was listed and labelled for its use. The employer was cited for a violation of the same standard with similar hazard on January 30, 2020, Inspection 1439223 #, date February 27, 2020 of final order/settlement, and means of abatement.

Notice 2#, Item 5#, 29 CFR 1960.58 as required by 29 CFR 1960.8(b): The employer failed to ensure collateral duty safety officers received the necessary training for their assigned duties. The employer was cited for a violation of the same standard and hazard on June 17, 2019, Inspection 1403251#, date July 2, 2019 of final order/settlement, and means of abatement.
