29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about October 23, 2020, the employer failed to ensure that employees exposed to flying particles had adequate eye protection. Employees performing framing work were using nail guns and were exposed to eye injury hazards. Employees did not have eye protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 23, 2020, the employer failed to ensure that employees engaged in residential construction activities 6 feet or more above lower levels had adequate fall protection. Employees were performing framing work on a residential structure and were exposed to falls of 12 feet. Employees did not have adequate fall protection.  The Rock Creek Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1210751, citation number 1, item number 1, and was affirmed as a final order on 4/11/2017, with respect to a workplace located at 6555 Stoney Ridge, Medina, OH, 44215.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about October 23, 2020, the employer failed to ensure that the ladder used to access the upper landing surface extended at least three feet above the upper landing surface.  The Rock Creek Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(4), which was contained in OSHA inspection number 1210751, citation number 1, item number 2 and was affirmed as a final order on 4/11/2017, with respect to a workplace located at 6555 Stoney Ridge, Medina, OH, 44215.
