29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 22, 2020, and at times prior thereto, the employer did not ensure each employee in an excavation was protected from cave-in hazards. This condition exposed the employees to being crushed and engulfment hazards by trench wall cave-ins. GRod Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1390896, citation number 1, item number 2 and was affirmed as a final order on June 25, 2019, with respect to a workplace located at 102 East Highway 544, Murphy, TX 75094.

Exposing them to crushed by and engulfment hazards.
