29 CFR 1910.178(o)(1):The employer did not ensure that only stable or safely arranged loads were handled: (a) Facility, MAG Shop: On or about October 21, 2020, the employer did not ensure that the A5 Test Fixture was stable and safely arranged when it was picked up by the Komatsu forklift. The A5 Test Fixture rotated approximately 90 degrees when it was picked up by the Komatsu forklift in the Weld Shop of the MAG Shop. The Komatsu Forklift Operation & Maintenance Manual for this forklift requires that a pallet be under the load that is being picked up.
