29 CFR  1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval.   a) Fuel Pier, Building 521 - On or about August 24, 2020, employees operated powered industrial trucks to lift and move 125 feet section of fuel pipe,  attaching a nylon choker to the forklift tines which was an addition affecting the safe operation of the equipment and were made without the manufacturer's written approval, exposing employees to struck-by hazards.
