29 CFR  1926.600(a)(3)(ii): Equipment parked on an incline did not have the wheels chocked and the parking brake set:  (a) 394 Heisey Quarry Road, Elizabethtown, Pennsylvania - An employee connected the 2013 CAM Superline flatbed trailer (Trailer 3856) to the 2007 International 9200i truck, VIN 2HSCESBR97C494066 (Truck 4066) at the staging area, which had a downhill slope toward Heisey Quarry Road.  After connecting the trailer, the employee got into the truck.  The employee then exited the truck with the truck's engine running, and the employee did not set the parking brake at this time.  The truck and trailer combination started rolling forward down the property's driveway with the employee out of the truck, exposing the employee to struck-by and caught-between hazards, on or about October 28, 2020.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
