29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about October 30, 2020, employees conducted roof work on a flat portion of a residential building without the use of a fall protection system. This exposed employees to a fall hazard of approximately 12 feet to the lower level.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  (a)  On or about October 30, 2020, employees conducted roof work on a flat portion of a residential building without the use of a fall protection system. Employees were not provided with training on the use and limitations of fall protection and how to eliminate or minimize fall hazards. This exposed employees to fall hazards of approximately 12 feet to the lower level.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  On or about October 30, 2020, employees conducted roof work on a residential building and utilized a ladder for access and egress, that did not extended 3 feet above the landing. This exposed employees to fall hazards of approximately 12 feet to the lower level.
