29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Aquaflex Press Room: On or about October 29, 2020, employees performed maintenance and servicing on various plant machinery that has multiple sources of energy, but specific deenergization procedures had not been developed, documented and utilized for the control of potentially hazardous energy of the Aquaflex Press Machine.  Note: Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section and shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to those outlined in (c)(4)(ii)(A) through (D) of this section.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Production Work Area, Line 75: On or about December 14, 2020, employees operated a Farason Zecuity Foil Fold & Punch Machine, where machine guarding did not prevent the operator or other employees from accessing moving machine parts while operating.

29 CFR  1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:   (a) Aquaflex Press Room: On or about October 29, 2020, the Aquaflex Press Machine  guarding  was not adequate to prevent operator access to moving machine parts while operating the machine. Employees are exposed to moving machine parts while cleaning cylinders.
