29 CFR  1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Joshua Fine, on or about October 30th, 2020, at the location of Sublot 110 Gladstone Mill Dr., North Ridgeville, OH, 44039:  Employees were performing residential roofing work without the benefit of accident prevention programs to reduce and mitigate worksite hazards. Worksite hazards unprotected against included fall hazards of approximately 11 feet while working on the roof and accessing the roof using a ladder that was not set up appropriately and the appropriate employee training for recognizing and mitigating these hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)   a) Joshua Fine, on or about October 30th, 2020, at the location of Sublot 110 Gladstone Mill Dr., North Ridgeville, OH, 44039:  Employees were using a portable ladder to access a roof to perform roofing work that extended only one rung above the roof, exposing employees to potential fall hazards of 11 feet while accessing and exiting the roof.
