29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about November 3, 2020, at a construction site located on Pavoni Lakes Ln, in Humble, TX, employees were exposed to eye injury hazard while operating nail guns without wearing eye protection.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about November 3, 2020, at a construction site located on Pavoni Lake Ln, in Humble, TX, an employee was exposed to a fall hazard of more than 6 feet while placing wood slabs on the roof of a house without the use of any fall protection system.
