29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) At the  commercial jobsite, an employee who was working from an aerial lift was not wearing body harnesses with a lanyard attached to the boom or basket. An employee was exposed to falling 25 feet to the ground below.

29 CFR  1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  (a) On or about October 30, 2020 an employee was working from an aerial lift were provided with a Miller T4000-UAK yellow harness. The harness contained defects caused by the d-ring and cam buckles and other hardware having excessive corrosion and deformation exposing employees to the hazard of falling 25-feet to the ground below;  (b) On or about October 30, 2020 an employee was working from an aerial lift were provided with a Werner Upgear harness. The harness contained defects caused by the cam buckle and other hardware having excessive corrosion and deformation exposing employees to the hazard of falling 25-feet to the ground below.
