29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  a) Exterior, Roof of the house: An employee was exposed to fall hazards of approximately 25 feet, while engaged in roofing activities of a residential home without any means of fall protection in place, on or about 9/22/2020.   b) Exterior, Garage roof: Employees were exposed to fall hazards of approximately 12 feet, while engaged in roofing activities on a garage roof of a residential home without any means of fall protection in place, on or about 9/22/2020.  One of the exposed employees was a 13 Year old minor boy.

29 CFR  1926.1053(b)(9):The area around the top and bottom of ladders was not kept clear:  a) Building Exterior front: employees were exposed to tripping hazards while climbing down from an extension ladder that the bottom was not kept clear, on or about 9/22/20  Employees were using packs of shingles to prevent the ladder for sliding.
