29 CFR 1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) Whittemore Company, Inc. located at 30 Glenn St., Lawrence, MA, 01843 On or about November 6, 2020, employees were provided voluntary use of N95 disposable respirators (Moldex, 2700 N95) for blending and/or bagging processes, but not provided Appendix D advisory information regarding employees' proper usage.
