29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards while operating a powered industrial truck without a seat belt being used to protect the operator:  a.	On or about Nov 6, 2020, in the warehouse area, employees were operating a powered industrial truck that did not have a seat belt installed for use, thus exposing the operator to crushing hazards.
