29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  Crabapple Market Place East Parking Lot located at 12650 Crabapple Road, Milton, GA 30004, Excavation: On or about 11/06/2020, the employer exposed employees to cave-in/entrapment hazards while working in an excavation that measured 60-feet long by 10-feet wide by 5 to 6-feet deep, in that a ladder or other means of egress was not provided within 25-feet of the location of the employee installing 8-inch ductile sanitary sewer lines.

29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:   Crabapple Market Place East Parking Lot located at 12650 Crabapple Road, Milton, GA 30004, Excavation: On or about 11/06/2020, the employer exposed employees to cave-in/entrapment hazards while working in an excavation that measured 60-feet long by 10-feet wide by 5 to 6-feet deep, in that the spoil pile was not kept at least 2-feet from the edge of the excavation which created a deeper excavation as the pile was approximately 6 to 7-feet high.

Crabapple Market Place East Parking Lot located at 12650 Crabapple Road, Milton, GA 30004, Excavation: On or about 11/06/2020, the employer exposed employees to cave-in/entrapment hazards while working in an excavation that measured 60-feet long by 10-feet wide by 5 to 6-feet deep, in that a cave-in protection system was not provided to an employee engaged in pipe installation activities.
