CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     On or about November 06, 2020 and at times prior thereto, at a workplace located at 5351 W. Sublett Road, Arlington, Texas; the employer did not provide training to forklift operators to ensure employees are competent to operate the truck safely. The employees were exposed to fall and rollover hazards.

Sublett Road, Arlington, Texas; employees working from a basket were not provided fall protection. Employees were exposed to fall hazard of approximately 18 feet to the ground below.

Sublett Road, Arlington, Texas; at the new residential construction site, employees were walking and working from the roof and not protected from falling more than 18 feet to the lower level by use of guardrail or personal fall arrest system.
