29 CFR  1926.451(b)(1)(ii):The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm):  On or about November 9, 2020, two employees were working from an approximately 25 foot high scaffold platform that was planked only 18 inches across a 60 inch scaffold endframe.

29 CFR  1926.451(c)(1)(ii):Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide.  On or about November 9, 2020, two employees were observed working on an approximately 25 foot high fabricated frame scaffold that did not have guys, ties, and/or bracing installed.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about November 9, 2020, two employees were observed working on a 25 foot high work platform of a fabricated frame scaffold without any fall protection established in CFR 1926 451(g)(1)(i)-(vii).
