Middletown, NY - on or about November 07, 2020, an employee engaged in rigging operations was exposed to a struck by hazard when he was not provided with adequate training to identify hazards associated with free rigging operations.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)2278 Goshen Turnpike. Middletown, NY - on or about November 07, 2020, an employee that was in close proximity to roof trusses that were being lifted into place by a forklift was not wearing a hardhat. The employee was exposed to head injuries while helping to guide and secure the trusses into place.

Middletown, NY - on or about November 07, 2020, The employer was utilizing a Genie GTH 844 all-terrain telescoping boom/tele-handler, forklift moving an unstable and unsafely arranged load (truss) that toppled over and fatally struck an employee.

Middletown, NY - on or about November 07, 2020, Synthetic slings used to lift loads, including but not limited to wooden trusses, were missing permanently affixed and legible identification markings that identified the safe working load limits as prescribed by the manufacturer.
