29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the opening.   a)  On or about November 11, 2020, located on the east side of the school, employees were exposed to fall hazard greater than 6 feet when performing roofing tasks without protecting a roof opening with a cover, guardrail system or personal fall arrest system.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)  On or about November 11, 2020, located at the jobsite, employees performing roofing work were exposed to fall hazard greater than 6 feet when using a warning line system without a combination of different fall protection systems.

29 CFR  1926.502(f)(1)(ii):  When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation.  a)  On or about November 11, 2020, located on the east side of the school, employees performing roofing tasks were exposed to fall hazard greater than 6 feet when the warning line system did not extend to the wall.  b) On or about November 11, 2020, located on the east side of the school, employees performing roofing tasks were exposed to fall hazard greater than 6 feet when the warning line system was not set-up at the proper distances.
